A smaller, more rural county anchored by two distinct cities: Niagara Falls and Lockport.

Niagara County is home to 212,666 residents as of the 2020 Census — well under a quarter the size of neighboring Erie County. Its geography is noticeably more rural and more spread out, without the single dominant hub that Buffalo provides for Erie County. Instead, Niagara County is anchored by two distinct anchor cities: Lockport, the county seat and an Erie Canal-era small city built around the historic "Flight of Five" locks, and Niagara Falls, the tourism-driven legacy industrial city built around the falls themselves. North Tonawanda, on the Niagara River across from Erie County's Tonawanda, rounds out the county's third urbanized area.

Niagara Falls Memorial Medical Center is the county's anchor hospital, and it operates the Schoellkopf Health Center skilled nursing facility directly — useful for a patient discharging from that hospital straight into skilled nursing care without a transfer. Niagara Hospice, headquartered in Lockport, has served county families since 1988 and operates its own inpatient Niagara Hospice House rather than offering only in-home hospice.
